Required Athletic Paperwork

FWJH has partnered with FinalForms, an online forms and data managment service, to process all athletic forms. Incoming seventh graders and all new athletes who will compete for the first time at Flowing Wells Junior High School, must have a current physical examination and complete all the paperwork in order to be eligible to participate in athletics. Returning athletes who already have a physical examination on file, must update their FinalForms profile in order to be eligible and participate in athletics. If you do not have personal insurance, the school does offer insurance for your athlete that is available for purchase. Please contact the office at 520-696-8550 for more information.
